9/10/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a jovial mood evident by her facial expressions as well her overall communication with QMHP-C. QMHP-C inquired about her day. Ashiyah verbalized to QMHP-C that her day was going well. QMHP-C informed Ashiyah that this session was going to focus on ways to control her anger in a controlled manner. 2. Behavior: Ashiyah presented cooperative behaviors without needing any redirection from QMHP-C during the session but within the session sometimes she was being uncooperative with her mother about what she needs to work on in an effort for bettering herself. 1.Intervention: QMHP-C began the session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C verbalized that the session would focus on anger management techniques due to reports from Ms. Hodge as Ashiyah has increased her attitude. QMHP-C modeled calming techniques and relaxation skills to support with stabilizing her moods. QMHP-C assisted Ashiyah with identifying her triggers and fuses that causes her to become upset and frustrations. QMHP-C reiterated journal writing and problem solving techniques. 2. Intervention: QMHP-C processed on journal entries with Ashiyah as she struggled with topics that helps to express her thoughts and feelings. QMHP-C engaged Ashiyah in a therapeutic activity as it relates to conflict resolution and how to handle her anger triggers. Throughout the session, QMHP-C processed on age-appropriate decision-making, along with building effective communication. 1.Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Reports from Ms. Hodge revealed that Ashiyah had been displaying a lot of attitude evident by being disrespectful and talking back. Ashiyah reported that discussed with QMHP-C that she struggles with her anger and that it is difficult to calm down when upset. During the session, Ashiyah verbalized to QMHP-C that she does struggle with identifying her triggers. 2.Response: Ashiyah reported that she has difficulty with journal writing as she feels that she doesn't know what to write and that she doesnât like writing she just wants to handle the situation when it occurs. During the therapeutic activity, Ashiyah was able to provide QMHP-C with positive ways to handle conflict as she discussed with QMHP-C that she doesnât utilize those techniques.
